I have tried numerous under garments and none work.

Get several leak-proof pads for him to lie on. I think they're called "Bucs"?? You can get these pads with one-use disposables or the Bucs? pads are washable and reusable.

"Chux" is a brand of disposable bed pads, personally I used washable ones as they are much larger and stayed in place better.
Uhm, have you tried to make sure he's got everything tucked in toward the opposite side?

be sure to line the entire mattress with a protector. The plastic over the mattress makes cleaning easy. Urine cannot get through.

Plastic pull-ons over the diapers may work, if they're put on right and if the pull-on is the right fit. We also had an easy-to-remove waterproof mattress cover. And pads. The disposable ones were useless with my husband, always in a ball under or beside him. Get good pads, big enough to turn over on, and enjoy the freedom of not laundering multiple sheets a day.

I suggest you find a good local medical supply store with excellent staff who know their incontinence products well. There are other brands available at these stores that are much better than the kind you find a Walmart and CVS.

This is a multiple point attack solution but becomes very easy.
As others have suggested, start with a good quality water proof mattress cover. The good ones are not the old crinkly vinyl or plastic that we used for our kids. Target sells a really good one at a realistic cost and this is a “once every 2-3 year” expense anyway.
Then the first line if attack would be his nighttime underwear or diapers. The diapers hold more but if the “diaper “ thing bothers him, the disposable underwear at Sam’s (Member’s Mark brand) is the best and least expensive we have used, and we have used every major brand. Admittedly ours are for my wife but they are the same product.
You can supplement the holding quality of the disposable underwear with the use of “booster pads”. They hold. lot of liquid but will let any excess flow through to the nighttime underwear or diaper.
The next and last is the washable bed pad. As someone has recommended, get a good brand, made in America. They will have a waterproof side that is “breathable “ and comfortable. They are also strong enough to be used as a “slide sheet” if the person is not very mobile themselves. These good ones are washable up to 209-300 times commercially so probably more in a home environment. A set of (4), 34” x 36” is less than $30. You can get 36”x 54” also at a slightly higher price. We use the Linteum Textile Company brand with a Vintex backing, all American made and good stuff and heavy enough to stay in place. Free shipping from their web site. That was what our local hospital uses and where I got the name.
I promise you, with these steps in place, no more wet sheets, no more stripping down, wash and remake the bed each day.
One last thought. My wife was a side sleeper and with the use of a large foam rubber wedge placed just at her side from a prone position, slipped under edge of the bed pad (very important) she has gotten to where she just stays flat all night. We had to change her sleeping habit because of it irritating the cartilage in her ear from the pressure all night but not thatnis no longer a problem for her or me.

I bought my stuff through Amazon. I found a cotton top full mattress pad then I put a washable bed pad on the area that he sleeps on that may get wet. I cover that with the fitted sheet and then I put another washable pad 36x 52 , I think , on top of the sheet. He sleeps with a nighttime diaper on which only leaks if he sleeps on his side. The only thing I have to change is the top washable pad which I have a spare to put in place of the wet one during the night. He sometimes will remove the top pad and if he does the other pad beneath the sheet catches the urine. I then have to change out the sheet and the pad under it but not the whole bed.
