You might also consider a visit to an elder law attorney. A POA is reimbursed out of the principal's funds for this expert advice.

What you need to do is what Geaton has suggested, go to your County Social Service Office and speak to a Social Worker there. That person will help you thru the Medicaid application and tell u what you need to do.
If your ex is hospitalized, tell the SW there you can no longer take care of him and he needs to be placed. To release him to home would be an "unsafe discharge". You refuse to pick him up. If you have no POA, tell the SW that the State will need to take over his care. Walk away, don't answer ur phone.

What do you mean “ we don’t do that “? All the Dr.’s office needs to do is send a referral to the social work department that is connected with his network. It’s very simple. There is a social work department at every major hospital in your area. His comment doesn’t make sense. But if that’s really the case, your next step would be contacting your local Agency on Aging, or if you don’t get help from them, contact your state’s elder protective services.
You contact Social Services for the county where your ex resides (usually the Dept of Health and Human Resources).
If you aren't the PoA or legal guardian for your ex, you may have to report them to APS as a vulnerable adult if your ex doesn't (or can't) request the social worker themselves.

In this scenario they will begin managing all their affairs and find an appropriate facility for him/her and then your ex will have all their needs met.
