How can I help my grandma deal with her arthritis? She's now being diagnosed with a heart failure disease.

She has lost the ability to walk since she's weak please help me I don't wanna lose her.

You need to google CHF or Congestive Heart Disease. Her inability to move around may be a combo of arthritis and breathlessness. Heart failure is basically a heart that is failing as a pump due to age and disease. It has no cure. Symptoms of fluid in lungs and in peripheral extremities can be addressed with medications, but a failing heart pump indicates that this is no longer a 29 year old heart. It is older and more tired.
Is your Grandma now confined more to a wheelchair. I am so sorry to hear this. With good medications she may have some quality years left, but as yet Medical Science has found no way to take us back from aging to youth. She is lucky to have a caring and loving granddaughter to help her now.
