Medical Information Release form example?

Most places have their own form.
So if you want information from your "old" Primary sent to a new doctor the new doctor's office will have a form, or you can go to your current doctor and request a release of information.
It might be possible to send a letter asking that the information be sent to you and this might be enough of a release but I think in this day and age of "litigation" the office will want the request on their form.

Many forms also want you to indicate what records are being requested. Is it just visit notes, lab results, and from what date to what date.
Also know that there may be a fee involved for each page of information.

The best way to figure out what they need to release information is to call the office and ask for a form to be sent to you. (they may even have one you can do on line)
