Most (all?) states not only have an agency for aging adults, but also agencies for people with various kinds of handicaps. My state has a single-entry website to access any of them. Since your friend is only 50 they may have more suitable suggestions for her.
Many adult day health programs (adult day care) have older populations, but it would be worth checking to see if there are any geared to younger folks in your area. That provides various activities and sometimes outings. She could choose to go one or more days a week.
Check local hospitals for stroke support groups. They may not only meetings, but activities as well.
And depending on her mobility, the local Senior Center might be a good choice. Some are very active, others (like the one in my city) are mediocre and just seem to plan casino trips.
