What can I do when I cannot get mom's clinicals forwarded to a new location?

My Mom came to this place 17-18 days ago from the hospital. I have tried for over a week to get her clinicals forwarded to the new location. They have received some info but not all each day! I faxed a request for transfer, and have probably left 2 messages daily for this lady. She has not returned 1 call! Is she trying to keep Mom the 20 days, she's the one who told us she could be transferred. What should I do now call the state or something?


Mom was sent to this care home from the hospital, but I asked for transfer to another that I feel is better. The social worker told me this was ok but she will not forward her records/clinicals to the other location. I have fax the request, sat and waited to ask her to send them, she returns NO calls, the S W from the other facility has requested them and left messages so my mom can be transported there. Moms 20 days are up soon, do you think the just trying to ride his out?

Peewee, I'm reallly confused. Your mom was in one facility and you requested she be transferred to another facility? What person are you trying to reach at facility 1 and what records are you requesting be sent to facility 2? .

Yes I have POA, we met with the department heads Tuesday of last week and requested another facility that day. She gave me the application to apply for Medicaid, she faxed something but not all records needed, only then would the other location accept Medicaid app but since then she's in meetings, NEVER return calls or messages and the receptionist says she forwarded them to her, she don't answer her phone, the other facility has sent her a list if what's needed, no response or return call. Is this Ombudsperson at a certain location what is that? Thank you!

Does your mother have a POA or DPOA allowing you to request records or information on her behalf? If not, the social worker might be refusing because there's no legal support for providing information to someone w/o written authority.

If you do, then I think she's either stalling, or it's not her job. The facility may have a medical records department, or may not. Find out from the Administrator, and explain when you speak with her that you're not getting response from the SW. If the Administrator doesn't commit to providing records, contact the Ombudsperson for your geographic area.

Doctors office did not send info on my husband to surgeon. Told them I was coming the next day to pick up all the materials at their cost to copy. They faced that day.

Who are you calling at the hospital? It would be medical records department, not the discharge office.

Are you saying that mom's hospital records have not been forwarded to the rehab facility? It's the facility"s job to request, i think.
