Dad alternates beween peeing too much and being unable to pee. Lost sense of environment. What to do?

At night, he needs to pee too much or not at all,tho the urge is there, Last night he didn't know where he was(in his bedroom).I don't know who to speak to or what to do,other than call his doctors.
