It sounds like bit of aphasia may be at work here. I had to play the game with Luz (DW). She finally quit talking because her comments made no sense and we could not communicate or her replies might be as much as five minutes late. I would just talk and ask question and watch her face for reactions to get my best answer. That worked most of the time. As an example of her talking going wrong. We were in walmart once and she ask me if she should go to the power. I thought just for a second ask her if she needed to go to the bathroom. She said yes and we off to the races. Luz eventually quit talking altogether.
Just have patience and say something and eventually the comment will make sense. Pay attention to what they are doing or watching on tv and generally you will get it. It took me some time to figure this out and most of the time I can get it but when I don't I try to introduce a new topic that is familiar to him. I have been doing this for awhile now and I know how frustrated you get but you have to have loads of patience and when you feel overwhelmed go to another room or outside. My husband thinks I go to the bathroom a lot because he doesn't follow me there and when I come back the conversation has taken a turn. Good luck and stay strong.
