My grandparents were discharged from nursing home and sent home with another patient's medication.

You contact the facility and also report to the state DPH. My mom picked up a script which was double the dose for her diabetes. Her sister called me. I filed a report and they investigated. The pharmacy was fined and the pharmacist was required to to take 2 hours of classes on diabetes so that she did not lose her license

Yes, you need to report this to the discharge nurse. BIG mistake. Does the bottle have someone elses name? Then the fault of the staff members. Dads name with wrong med, the pharmacy's mistake and a really BIG one. My daughter has seen pharmacies fired because of a mistake like this. (Nursing homes do not usually have pharmacies on site, they contract with off site pharmacies) You need to call because they may have given Dads med to another patient.

I would recommend calling facility and reporting it right away. The facility should be responsible for fixing this as it is their mistake. Both parties need to get the correct meds. This is also a privacy/HIPAA violation.
