How do I request a transfer for someone who is in respite?

From the hospital, the nursing home that they moved him to isn’t very involved and isn’t providing services. We’d like to move to another nursing home that we know of where there is better support and it’s closer to home. We are not sure of how to do that. We know about looking at the waivers etc. for Medicare and Medicaid but we just need initial steps to get him moved.

Is he in the NH for rehab or as a long term care patient?

Have you called the NH you like to find out if there is a bed available and how to initiate a transfer?

Start with going to the institution you wish to transfer him to with all of the particulars of his case, his care needs and his medicaid qualifications. Ask if they will accept him. A transfer is simple if you are transferring someone to an accepting institution. A matter of calling transport.
