What are some soft foods for swallowing issues?

Since dysphagia in the elderly is not likely to improve modified diets are a long term commitment, so investing in a blender (like the magic bullet or ninja) is money well spent and will allow you to include almost anything imaginable.
For starters look to foods that might already be part of your diet:
mashed potatoes, sweet potatoes, squash, rutabaga
yogurt
custard
breakfast oatmeal and farina
polenta
refried beans
smoothies (consider adding protein powder)
silken tofu
applesauce
avocado
hummus
pudding
nut butters

If you don't yet need pureed consistency then things like:
rice pudding
grits
well done pasta casseroles
scrambled eggs
cottage cheese

Hopefully there has been a medical (usually done by Occupational Therapists or others trained to do this specialized eval) evaluation. Whomever did this will give you guidance on thickening fluids and on puree of foods and diet. If this hasn't been done, it's time to do it, and request a nutritionist for guidance.

My Moms esophagus is inflamed due to long time GERD. Dr is prescribing Prilosec twice a day for the next few weeks. He is scheduling a endoscopy in two weeks. I don’t believe this is a long time eating plan. Thank you for your input. It hadn’t occurred to me that this could be long term.
