I looked it up online and one definition is "an official or authoritative instruction".

Ones that we see discussed here relate to
a living will.
appointing a health care proxy
appointing a POA financial
The Five Wishes® and directives like it.
“Do Not Resuscitate” orders.

There are many examples online of directives for various situations, It would probably be wise to consult a lawyer to be sure you and your partner's wishes are clearly stated. I am sorry that she has been diagnosed with dementia. It is very sensible to get papers in order now.
