How is a "Restricted Application" filed for spouse Social Security?

I have read all the information about filing a Restricted Application for Social Security Spousal Benefits and excluding my own RIB so that it will be higher when I switch over at age 70. How exactly do I express that? There is no place on the application to say it is "Restricted". Thank you

Apparently, due to the law change, you can only do this if you turned 62 prior to January 1, 2016
