Does a Dr. or hospital have a duty to report to a medical POA when the patient has been declared incompetent?

I was POA at one time and was not notified of a test for competency and then wasn't given the results. If I had known, I would be Guardian now. Instead the friend who was given that info ended up being POA . Is that considered an E&O claim as it could end up causing me financial harm because I might have to pay medical and burial costs that I wouldn't have if I was in charge. I think it's a claim against both the hospital, doctor and POA.
