What is the criteria for hospice care?

My aunt is 83 with CHF. She has a pacemaker/defibulator and has trouble with fluid build up. She gets out of breath very quickly. She is mobile with a cane, however, not able to bath without help. She needs blood work on a regular basis to help monitor the medications she is taking.
