Moving mom from Brooklyn, NY to Smithtown, NY

How would I go about moving my mom to my home with Medicare involved. Can I get aides out here for her on a 24-7. she has severe dementia and is bed bound.

Sorry, posted too soon. If she is currently in a facility, the social worker can assist setting up medical transport, i.e., an ambulance or ambulette.
