How to explain cash withdrawals on Medicaid SC-1 form?

I am in Mass and in the process of submitting an SC-1 form for my mom who is Medicaid pending in a SNF. They want an explanation for all withdrawals from her checking account over $1k. There were 3 in amount of $5K, $3903, and $1K. Should I just make out a check for the total amount to the SNF and explain as used for private pay?

Babalou, the 1st w/d was for cash on hand for groceries, household purchases, etc instead of going to the bank. The 2nd and 3rd were to cover the cost and set funds aside for the previous SNF that mom was in prior to the current one.
