Brother expects the 2 of us to watch our mother (93) all the time at her house?

I work 33 hrs besides this. Mom has unstable gait and dementia. We've been doing this for almost a year now and I can't do this any longer but he won't take 'I can't'. He is retired and refuses home health aides and daycare. She can't afford long term nursing care. What can I do?
