My Mom's not able to keep doctor appt; not able to go out. In-home choices for doctors?

My Mom was discharged from Hospice, but still needs a doctor.   She has a lot of chronic abdomen pain, and some days does not get out of bed but for a few minutes. Can't make dr. appt. since we don't know when she will be feeling well enough. Anyone know of options for in-home dr. care? We need dr. to get her meds.
