My brother (50) and his wife have been living with my Mother for the past 15 years. Any advice?
He doesn't work, pay any bills and they abuse her.

If your mother is being abused, physically, financially or psychologically, report yhe facts to your local Adult Protective Services. Have you donevthis already? That's where i would start.
