POA and health care proxy going on vacation. How does he give live-in aide medical emergency consent?

I'm a live in aide for a 92 yo disabled women with dementia. She doesn't talk/understand anything. The POA is going out of town and wants to get me something in case of emergency if he's unreachable. I've tried googling it but only come across consent for minors.
Can anyone direct me or tell me what to do? thanks

That's weird bc her case manager said he could just write a letter and get it notarized but I was looking for a form. He's her guardian. He takes care of everything. It was just in case there was an emergency and I needed to call 911 or go to hospital. Oh well, the odds are slim but we just wanted to be safe.

The POA cannot give it to you, nor can a patient with dementia. Tell the POA to keep his cell phone on and be near a fax machine. Sorry, that's the law.
