At what point do you removed your elderly loved one from their home?

I am concerned about her being alone SO much. she is not very social anymore. She goes to church and also to the grocery store but otherwise is pretty isolated. I think that is causing her decline more rapidly. We have her home for sale and she has her name on a list for an apartment in a senior living complex. My Daddy passed away last July. We try very hard but are just not with her all the time.
