Mom is willing to address her hoarding issue, but the house is severe. Where can we find help with cleaning it and therapy for addressing her hoarding issue?

Mother is 86 yrs old. In hospital because of falling. Wants to return home -is willing to address hoarding to do so. House is severe. Looking for help with cleaning but also with cognitive therapy. She has bad heart and bad knee-will need walker. Mother lives in Montgomery AL-I am in NC.
