What care plan? Are you speaking of someone in a SNF or Nursing Home. Generally a person enters Assisted Living or Nursing Home after an initial assessment of needs. The initial plan then is in what this person needs. Are they independent on eating, dressing bathing? And etc. Taking meds? If a need for more care comes to the fore then in an Assisted Living facility it is generally the staff who reports to the facility that there is need for another level of care, which is charged for. In a Nursing Home some protocol will be in place. So it would be a very good idea if you asked for this information at the individual facility you are dealing with. Independent and Assisted Living Facilities are usually facing less state regulation and rules than say a Nursing Home or a SNF. Ask at your own facility. Everything varies institution to institution and state to state.
