Consult with an attorney. January of 2018 the NC Uniform Power of Attorney Act was adopted. So, documents after that date would need to comply. Old forms may not.

I live in NC and when I needed a medical and durable POA for my husband, I just had our lawyer draw up both for us. That was back in 2017, and they cost $100 each. Because it was difficult for my husband back then to get out, the lawyer came to our house for us to sign them. Otherwise I'm sure you would have to go to the lawyers office to sign.
