Preciosa7, many of us here on the forum have gone through something similar. Most of the time we needed to wait until there was a serious illness or a major fall, where 911 is called, the person stays in the hospital for whatever length of time .... then is transferred to a Rehab Facility.
If the person doesn't progress in the Rehab Facility, then Rehab would recommend a higher level of care. Thus a "nursing home" or "Memory Care".
My own Dad was living in a senior facility in "Independent Living", and it was the facility, not a doctor, who recommended it was time for my Dad to move to their "Memory Care". One doesn't need to be formally diagnosed. The facility usually does their own evaluation.
As JoAnn29 said it depends on whether you're his PoA and whether your authority is currently active.
If no one is his PoA the most you can do is contact APS for his county to report him as a vulnerable adult and get him on their radar. They will eventually acquire guardianship of him which will legally enable them to transition him into a facility -- he doesn't need to be cooperative.
In the interim, if he has a medical emergency that requires hospitalization, you can contact the hospital social worker to inform them of his situation so that they can perhaps have him go directly into a facility from either the hospital or rehab.

As the POA you can place him, you have the authority. No POA it will be harder. A Dr. can write an order saying Dad needs 24/7 care .
