You posted this under "end of life". I am thinking perhaps you want a PDF for advance directive for your state? This can be found online with "living will state of.............." or "advance directive state of............'. Do know these can be added to if you have other things that need to be stated in black and white about what will be accepted and what will not. For instance I put, at a certain age, I don't want any feedings by tube, either IV or NG or PEG and that I will not accept dialysis, even temporarily.
