What do I do? My father's dementia is getting worse. He has no savings. It's getting harder to care for him at my home.

Please, please look into Medicaid with either a lawyer who specializes in elder care or a social worker who has success helping people apply for Medicaid. It pays for home care, even for family members who do the caring. Google it.

Please note a lawyer probably will cost money. Also, finding a Social Worker to talk to may not be free or available. He should call DHS or the Department of Aging in his city.

Contact your State’s Department of Human Services and apply Medicaid. With DHS you can determine if you can get help at home with Dad or find a Nursing Home to take care of him. Call 311 if you need additional help.

How much SS does he get each month? I would consult with an elder law attorney or Medicaid Planner. In TX what does Medicaid pay for? In some states it will pay for some AL, but in others only LTC. If you are not his PoA you can allow the county to become his guardian and then they will deal with him.

Sounds like your dad's condition is very difficult to deal with at home. You sure are doing a lot for him and he's not able (or willing?) to do much of anything for himself anymore.

Find a local nursing home that takes Medicaid and start the process of applying and getting him into the facility. If that doesn't work for some reason, use his money and hire some aides to relieve you a bit.

Your profile states that you are a physician. Surely within your professional circle there must be people who can well advise you on how to get your impoverished father on Medicaid and into a care facility.
