How do I find a memory care facility for a combative vascular dementia patient?

My uncle is 65 years old and been diagnosed with vascular dementia after two strokes in less than a month. He was discharged from a nursing home saying that he was high functional and can live on his own. I believe that he was discharged because he was being very competitive with staff and other patients. He had a psych evaluation and that's when they found out that he had vascular dementia. We live in Indiana and I'm having a problem finding a facility that can meet his needs.

Also, is his combativeness being addressed? I suppose the only answer to that is medication?

They have him on seroquel 50mg for night time and that has helped the combativeness.

Sorry for your troubles. Is he living with you?

I'm surprised that a nursing home sent someone home. Seems like they're usually more interested in keeping people there and keeping the money coming in. But anyhow, did he have the psych eval before his discharge?

What are his needs that most facilities are unable to meet?

Yes he had it a month before he was released and they said that his combative nature is due to the dementia and the damage done to his brain from blood clots. Since his record has that he is combative no facility wants to take him. They can meet all of his needs except the combativeness.
