Mother (91) is being cared for by mentally challenged sister and insists they don't need help. Any suggestions?

What kind of help are you offering?
Most people appreciate a gift of food, flowers, money for necessities.
Not so much unsolicited advise or criticism. Not saying you are doing that. You didn’t give enough information to know what the problem is.
If you feel that someone is in harms way perhaps you could contact your Area Agency on Aging and ask that they be checked on.
Edit:
I flashed on a family from my childhood. There was a handicapped daughter. Downs Syndrome I believe. That mom and daughter live together and the mom is probably about 90 now. 
They might appear to need help now as they are older to someone who didn’t know them. I can appreciate that there could be a level of concern in such a circumstance but again, without more information, hard to help you.
