Where can I find an independent physician to examine and evaluate my father?

My father is currently in a facility in Sacramento, CA, and we are moving him to our state, Puyallup, WA facility. We are in need of a examination and evaluation of his condition, he has Parkinson's and Dementia, and what kind of care he should be receiving in WA. We need help ASAP, as we are moving him up on Nov. 19th, 2017. Please help us, I am overwhelmed by all of this. THANK YOU IN ADVANCE!
