What necessary actions need to be done to simply change the fiduciary in charge of my Mother's VA affairs?

The individual that is currently serving as my Mother's fiduciary is no longer capable of filling the responsibilities due to an unfortunate car accident she was in. I simply want to change the fiduciary person....I'm a sister and I have everyone's blessing. Is this doable? Do I have to go to the nearest facility? Do I have to go through the selection process again?
