If a spouse is a caregiver, does payment for the care add to our household income?

If you are getting paid by Medicaid etc. for health care provided to someone else under a waiver program, then yes, it counts as additional household income. It may not be taxable for federal income tax - check with a tax preparer - but it is included for benefits calculations.
