Happens in the evening. Otherwise, fairly alert and in control of conversation? What type of Dr handles this type of assessment. Mom is 88, seems very alert, normal, but big fears enter mind in the evening recently that do not exist. How can she be assessed and is there treatment?

A behavioral neurologist would also be a fine choice.
Does she have a primary care provider? If so, perhaps starting with a thorough examination would be good. If she doesn't have a PCP, try to find a geriatrician.
Yes, delusions can be a dementia symptom, but that is not the only possibility. Does she have hallucinations, too? That is, does she see or hear things that aren't there? How about her sleep -- any disturbances once she goes to bed?
