Is there a service provided that they can contact to have someone help with this? To have someone come to their home, assess any repairs/clean-ups required, market and sell the property, AND find them somewhere else to live plus also help them to pack up their house and move? I live an hour away and work full time with a 2 year old kid and wife and just don't have any time to do this for them.

I would suggest talking with a realtor you are interested in working with. I'm sure the realtor could access a lot of the services you are describing. Or work with a social worker if you are looking at assisted living or a nursing home. Depending on where they live maybe hire a caregiver to help coordinate this move.
