How do you get patient back into skilled nursing once discharged?

 Stroke after surgery affected walking, memory, etc.

I'll have to have him call his dr. I believe he needs assisted living. The nursing home refused to give me any information, even through I was in all his paperwork.

Does he mean that he wants to become a permanent long term care resident? Would an Assisted Living facility be the right level of care for him? His doctor could order an assessment to determine this.

Medicare does not pay for long term care. They DO pay for rehab. Was he discharged because he met his therapy goals, or because he was no longer making progress?

He had aneurism surgery. Stroke after surgery. In hospital 5 months. Moved to skilled nursing facility. Was there 30 days and discharged. Now has visiting nurse and physical therapist come in to apartment. Has difficulty walking, taking care of self plus some memory issues from stroke. He asked me today is there anyway he can go back to skilled nursing facility. He has about 60 days Medicare and supplemental insurance coverage.

Im unclear what you're asking. Is this person currently hospitalized? In that case, you work with discharge planning.

Is this person at home? Are they "at need" medically for a NH?
