Where to get a trust for elderly parent with brother as POA lawyers ects?

Like my br can just  open up bank acct read some answers but really dont unst. I guess would be more of irrevocable trust as she can be penalized from Medicaid as she makes spendown but can put some money in trust. For her needs??? She could pay for private room etc
