Your state or county court system may have the form you fill out to apply for Guardianship. It is a judicial process and most states also require Guardians undergo state mandated training to be a Guardian. Look up probate court for your particular county.

You can find forms for POA, is that what you need? Guardianship is not a do it yourself deal, you need to petition the court for a formal guardianship.
