Why is my discharge from this nursing home taking so long?

I have been admitted to Autumn Lake Health Care in Cromwell CT. I am 46 years old. I Was admitted here back in September of 2014 for an abscess on my buttox and for controlling my diabetes. I was origally told by the social worker that I was only going to be here for a maximum of only 2 to 3 weeks. Since then i have been told twice that i was being discharged and all they do is keep delaying it. They signed me up with "Money Follows The Person" in February of this year. My brother has been appointed my conservator. I have just finish my physical and occupational therapy this week Monday May 11th. I have been told they feel i am completely independent and ready to leave from the therapists viewpoint. By the way i have been discharged from therapy 2 other times sice i have been here. I have been told that i would be discharged after dialysis has started but now i have been told that there are no plans for that right now. Money Follows the Person will not give me any information on any discharge plans and neither will the social worker. They are holding a big meeting on the last week of May. I think May 28th. I feel like I am getting the run around. It is hard to keep my hopes up when they keep delaying discharge. It seems like my brother does not share my point of view about discharge me to home care. Any help and advice would be appreciated. Thank You.

I am going to assume you are already on Medicaid, since you have been in the facility for 9 months. I don't see how you would manage dialysis at home, it's pretty involved. So Medicaid would have to approve home care and daily transportation to a dialysis center and back home. That would be really exhausting for you, and quite expensive. I'm going to guess Medicaid will go for the cheaper option.

I tried talking to a states attorney and they say they cant do anything for me. she just told me to try talking to the social worker again. It just keeps going around in circles. I feel like i'm trapped. No one will give me any indication how much longer they will keep me

I think it might be a good idea for you to contact your state's attorney. You might tell the supervisor of the facility what you intend to do and see if she'll hustle her bustle to dismiss you so that you can go home. Tell her that if you are not dismissed as promised, you will contact the state's attorney. And don't just say it; do it if necessary!
