Can a caregiver be paid in a non-Medicaid situation?

I am a social work intern who works with caregivers. I am in El Paso County in Colorado. A hospital social worker called me on this issue. I understand caregivers may be paid if the care recipient qualifies for Medicaid LTC-HCBS. One source on my search offered an outside possibility under Medicare but I can't find the schematics to it.

Medicare only pays for Medical care provided by licensed professionals--such as MD's, RN's, PT/OT/ST, etc. It never ever pays for caregivers to do things like housekeeping or take someone to the store. Medicaid in SOME states will provide caregivers and sometimes even reimburse family members, but Medicare never provides anything but licensed medical care. In fact the in-home Medicare PT provider for example, will be extremely careful not to do anything even remotely non-medical like take out the trash or make a sandwich. They are only there to provide professional services.
