Dad is very sick, trouble breathing, balancing, incoherent most times and can't see. Any advice?

Mom is in SERIOUS denial. I'd like to take him to the hospital, but she says No. I've noticed some things she had not done also like pay her bills or leave the stove on. How can me and my brother (forcibly take over) power of attorney?
