Many years mom named me on her Durable POA. She now has dementia. Who makes the decision that she is no longer capable?
Do I need documentation from doctor that she is incapable?

You would need documentation from a psychiatrist appointed by the surrogate's court to declare her incompetent and they will appoint a guardian.
