How to I get Medicaid for a California senior that wants to move to Plainfield, New Jersey?

This senior is 70 years old and is currently living in a nursing home in Fresno, CA but would like to move to a assistant living facility in Plainfield, New Jersey to be closer to her son. Her Doctor has deemed her to be fit to live independent with little assistance.
