Mom has hurt herself trying to assist dad and covers it up. What can I do?

At 92 Her rotor cuffs are totally damaged and of no use & dad has dementia attempts to take himself to bathroom falls & can't get himself up calls her for help & she attempts to help or he can't pull-up his pants or get back to his chair
