Anyone have a Caregiver's spreadsheet they're willing to share? - Diabetes/dementia

I've hired two caregivers for my dad. Does anyone have a caregiver's spreadsheet they are willing to share that I can modify for them to utilize to track their care services they provide? A checklist is preferred to make it easiest.
