What's involved in a social services evaluation or home visit?

I asked for our Home Health provider's social service worker to come for a family meeting to discuss long-term options. An evaluation goes along any time they come out. After the meeting, she talked with Mother, basically to see how she was doing and if there were any Medicare provided items she needed. We had already purchased everything imaginable for her without Medicare's assistance. They have alot of information available and seem offer friendly assistance for additional services that might be needed by the patient and caregiver.
