Get the DOCTOR to order a hospice evaluation for him. It's not up to the MC to make decisions about when hospital beds are needed, but your husband's doctor! Putting a mattress on the floor is not done in the majority of decent MCs because that presents issues of its own! When hospice comes on board, along with a hospital bed, they can bring a bolster cover that is put on top which cradles the body in such a way to make it a lot harder to fall out of bed. Doing so cut my mom's falls down tremendously.

A hospital bed, with guard rails, could prevent broken bones from a fall. It's a no-brainer!
There are cushioned covers designed to cover open rails on hospital beds, when needed. Rails should be spaced appropriately in order to not cause a choking hazard. There is absolutely no reason for bed guards to not be used.
The facility should be problem-solving this: they can put his mattress directly on the floor, and if he had a hospital be they could raise and lower it; or you can get him a concave mattress that makes it difficult for him to get out.

In most states rails are no longer used due to choking/death hazard.
Rails should be spaced appropriately in order to not cause a choking hazard.
There is absolutely no reason for bed guards to not be used.
