Palliative care is a specialty, just like orthopedist, opthalmologist, gerontologist. You would be using a palliative care specialist, which if you have Hospice already is not necessary.

He needs to write the order and then you will go through an "in home" agency near where you live. The doctors office should be able to give you a list of agencies.
If on Medicare or a Medicare Advantage they will cover the cost. Not Medicare age, then you will need to check with your private insurance to see what they cover.
Your question is too vague. Palliative care covers very little. No custodial care is covered. A palliative site might send medical professionals within the scope of their care and within the insurance provider network.
