Where can I find long term care for parkinson's disease?

Any facility that will meet the care needs would be appropriate.
If this person has LBD then probably Memory Care would be appropriate. If there is no diagnosis at this time with dementia then Assisted Living would be appropriate. If there are chronic medical needs that need to be met then a Skilled Nursing facility would be what you should look into. Any facility that you look into should evaluate your LO to determine what the appropriate level of care should be.
So start with the facility that you want.
If there is ANY possibility that your LO would have to apply for Medicaid later on make sure the facility accepts Medicaid so you do not have to go through the process of looking for another facility. And most places want a resident to be Private Pay for a time before Medicaid sometimes up to 2 years.

If this person is a Veteran look into the possibility that there may be Service Connected Disability and the VA may provide a little help or a LOT depending on where and when they served.
