Contact your local Social Service Agency and speak to a Medicaid caseworker. They have an "in home" program where friends or family can be paid for caring for you. Its based on income and how many assets you have. House is exempt.

jv, I am uncertain who you believe you are writing to, but we are just a bunch of caregivers from around the USA and a few other countries. We don't have forms. So I think, whoever you are hoping will pay your caregiver, you should contact their own website. Good luck.
